Short answer: medium-firm to firm, roughly 6.5 to 8 on the ten-point firmness scale, with a support core that does not let your hips drop below your shoulders. For most stomach sleepers that means a hybrid, or a dense all-foam mattress no softer than medium-firm. And your pillow matters nearly as much as the mattress.

Why stomach sleeping is hard on a soft mattress

Your hips and abdomen are the heaviest part of your body and, face down, they sit right in the middle of the mattress. On a soft surface they sink further than your shoulders and feet, so your spine bends into a U shape and holds that arch for seven hours.

That is the whole mechanism, and it explains why stomach sleepers so often wake with lower back pain that clears within half an hour of getting up. The fix is a surface that resists the hips specifically, not one that is uniformly hard. Firmness labels are not standardised, so treat the ten-point scale as a guide and prioritise the construction notes below it.

Firm is not the same as supportive

A cheap firm mattress is a hard slab with no give at all, which creates pressure points at your ribs and knees. A well-built firm mattress resists at the hips while still yielding a little at the shoulders and chest. The difference is in the layers, not the surface. The pillow mistake

Every guide ranking for this search covers the mattress and stops. For a stomach sleeper the pillow is half the problem, because your head is turned to one side all night and a thick pillow adds rotation on top of that.

What works: a very thin pillow, or none at all under your head. What also helps and almost nobody does. A thin pillow under your pelvis, which lifts the hips and takes the arch out of your lower back. It looks odd and it works.

If you use a pillow under your head, it needs to be flat enough that your neck stays close to neutral. Should you stop sleeping on your stomach?

You will read that you should. It is the position most associated with lower back and neck strain, and side or back sleeping is generally kinder to the spine. If you do switch to your side, the firmness target moves softer, and we set it out in what firmness is best for side sleepers.

In practice, people rarely change the position they fall asleep in, and lying awake fighting it costs more sleep than the position costs. The realistic approach is to make stomach sleeping less costly: firmer mattress, thinner pillow, pillow under the pelvis.
